Hormone Optimization: What Works, What’s Hype, and What Needs Medical Care

Hormone optimization has become one of the wellness industry’s favorite promises. A supplement supposedly raises testosterone. A particular food allegedly lowers it. A fasting protocol claims to reset the endocrine system, while a morning routine promises better energy, mood, metabolism, and libido.

These claims are appealing because they make a complicated subject feel manageable. Find the right ingredient, eliminate the wrong habit, and your hormones will supposedly fall into place.

The human endocrine system does not work that simply. Effective hormone optimization requires more than changing one food, supplement, workout, or laboratory marker.

Hormones interact with sleep, nutrition, body composition, training, stress, medications, age, reproductive health, and underlying medical conditions. The same intervention can produce different effects depending on the person and the context. Losing weight may support healthier testosterone levels in a man with obesity, for example, while prolonged underfueling may suppress reproductive hormones in someone who is already lean and highly active.

That does not mean lifestyle is irrelevant. Nutrition, resistance training, sleep, and stress management all contribute to overall hormone health. However, they should be approached as parts of a larger system—not as shortcuts capable of “balancing” every hormone.

A systematic review of randomized controlled trials found that calorie restriction increased total testosterone in three of four studies involving men with overweight or obesity.

Some people benefit from planned periods at maintenance calories during a long diet. These breaks may support adherence, training performance, and psychological recovery, but they should not be marketed as guaranteed hormonal resets.

The better question is not simply, “Does eating less lower testosterone?” It is:

Is this calorie deficit appropriate for this person, at this body composition, for this length of time?

Supplements and Hormone Optimization: Whole Foods vs. Extracts

Supplement claims often begin with a small piece of legitimate science and then extend far beyond what the evidence can support.

A compound may influence an enzyme in a laboratory experiment or produce a measurable effect in animals. Supplement marketers may then imply that eating a food containing that compound will cause the same effect in humans.

That reasoning can be misleading. Many supplement-based hormone optimization claims rely on laboratory mechanisms that have not been confirmed in strong human trials.

Curcumin, a compound found in turmeric, and piperine, a compound found in black pepper, are useful examples. Both have been studied for potential biological effects, including influences on inflammation, absorption, and drug-metabolizing pathways. Piperine is frequently included in curcumin products because it can increase curcumin absorption.

However, using turmeric and black pepper in a meal is not equivalent to taking concentrated extracts.

A supplement may deliver an isolated compound at a dose far higher than someone would normally consume through food. It may also combine that compound with ingredients designed to increase absorption. As a result, the supplement can behave differently from the spice in your kitchen.

This distinction matters because high-dose botanical extracts can interact with medications or affect how the body processes certain substances. Testosterone replacement therapy can be appropriate for men who have symptoms consistent with testosterone deficiency and repeatedly low testosterone levels confirmed through accurate testing.

Medical treatment may be part of hormone optimization, but only when the diagnosis has been properly established.

It should not be prescribed solely because someone is tired, getting older, struggling in the gym, or has one borderline laboratory result.

A more useful approach is to focus on factors that can genuinely support well-being:

  • emotional safety;
  • direct communication;
  • shared and independent interests;

There is no universal hormone-optimization routine. Still, the strongest foundation for hormone optimization is a consistent combination of adequate nutrition, resistance training, sleep, stress management, and appropriate healthcare. However, several fundamentals create better conditions for normal endocrine function and overall health.

Eat Enough to Support Your Activity

Chronic underfueling can interfere with recovery, reproductive health, mood, training performance, and daily functioning. Fat loss may require a calorie deficit, but the deficit should remain compatible with your health and activity level.

Maintain Muscle Through Resistance Training

Resistance training supports muscle, bone density, insulin sensitivity, mobility, and long-term physical function. It is valuable even when it does not cause dramatic changes in hormone levels.
